Home Healthcare, Hospice & Community Services has added Nicole Motus, LPN, to its hospice team as a Care Transition Specialist.
Motus will work with medical providers, long-term care facilities and community partners to educate families and staff about hospice services and quality-of-life options. She brings prior experience as a care transitions and liaison specialist with other hospice providers in the Northeast. She will primarily serve the greater Peterborough area, expanding access to end-of-life care resources in southwestern New Hampshire.
